How does it work?
To ensure the request is processed correctly, you will need to provide the following necessary details along with the wet signed hard copy of the consent letter.
- Forename
- Surname
- Middle name and maiden name(s) where applicable
- Father’s name
- Mother’s name
- Date of Birth
- Mother’s maiden name
- Nationality and country and place of birth
- Address
- Identification documents i.e. passport or national ID
Once we receive the report, it will be emailed to you immediately. Alternatively, it can be downloaded from our secure portal.
