How does it work?
To ensure the request is processed correctly, you will need to provide the following necessary details along with the wet signed hard copy of the consent letter.

- Surname
- Middle name and maiden name(s) where applicable

- Nationality and country and place of birth
- Address
- Identification documents i.e. passport or national ID
